The cost of the Application is $15.00 per applicant/submission* and must be charged to a credit/debit card or paid from your bank account, you will be automatically redirected to our payment site after submitting your application. You will be required to complete a "New User Registration" before making your payment. In order to obtain approval, supporting documentation as it applies to your situation including, but not limited to, the items shown below will be required. This information may be faxed to (717) 285-4504 or emailed to email@example.com:
- recent paystub from past 30 days
- any w-2's from prior calendar year or most recent federal tax return
- official documentation of other types of income (i.e. Social security, retirement, child support, etc.)
- photo identification
Please note all income supporting documentation that is submitted must be legible and clearly show applicants name, address, payer and reference date.
